SP4.2.5 Did COVID-19 change the way we treat appendicitis?

Abstract Intro/aims In repsonse to the COVID-19 pandemic, United Kingdom (UK) commenced a national lockdown in March 2020. Initial guidance advocated avoidance of aerosol generating procedures and hence we hypothesised there would be decrease use surgery treat acute appendicitis. Methods A prospective audit was undertaken across 4 hospitals South Wales, UK, during April Patients over 18 years age with suspected or confirmed appendicitis were identified data collected including their history, radiological findings, management, re-admission. Comparison made theatre from 2018 2019. Results total 254 patients treated 3-year period; 95 2018, 2019 64 During lockdown, Computed Tomography (CT) increased 36.3% 85.9% (p < 0.001). An antibiotic only approach appendicits rose 6.2% 40.6% Four cohort failed conservative required an appendicectomy. The overall rate laparoscopic appendicectomy reduced 85.3% 17.2% malignancy 3% cases. Conclusions Whilst less presented compared previous years, still significant reduction operation rate. aligns literature confirming antibiotic-only safe low failure It is essential however that are fully consented for this risk missed malignancy.
